The second question is easier. When is it worst?

Run your thumb along the inside of the strap and you will feel it. A moulded bar in clear frosted plastic, sitting in a sleeve along the length of the toe.
It holds the angle from minute one to minute fifteen, because a rigid moulded bar does not deform under the load a toe puts on it. Gel and soft silicone do. That is the whole difference between a session that ends where it started and one that does not.
The strap is the soft part. It pulls in or lets out to fit the width of your foot, so the pressure is steady rather than pinching. Firm, not tight. You should be able to sit through the session without thinking about it.
Strap it on, sit back, leave it alone. There is nothing to hold in place with your hand.

On the outside of the strap there is a small square screen with a green backlight and a set of buttons. That is the EMS unit. It charges by cable and runs the session.
It sends a low current into the tissue around the joint while the bar is holding the toe. What you feel is a light tapping or a prickle under the skin. Not a shock. Not heat.
Start on the lowest setting.
There are ten modes and the intensity moves up and down within each one, so the lowest setting is genuinely low. Almost everyone finds the first two or three sessions strange, and almost everyone stops noticing by the end of the first week. Move up only when the setting you are on feels like nothing.
